The pandemic has only heightened the vulnerability of countless children across India. We are looking for assistance to create a basic qualitative assessment form that could be administered across villages to help us understand the impact, child specific needs and gather individual/ family details required to link them to social welfare schemes

Thank you for responding so very promptly. At the outset let me share that I am not a tech person, but I have attempted putting down what I feel the requirements of the app/ form should be.
Yes, I am hoping this app/ survey form would be accessible on the mobile, as the data collection teams would not be able to carry desktops to the field. Also these villages are quite far away from towns to keep going back and forth.
I am attaching the data collection form we are aiming to circulate and collate data on.
Requirements
• Need to capture qualitative data
o The purpose of the survey is to (i) understand the impact of covid-19 on children across various age categories, and understand their needs (ii) list the individuals ( names, address, bank details + reason) in the villages who have no access to social welfare schemes, so that this can be presented to local administration to do the needful
o We did attempt using online survey forms (Survey monkey/ google form) – but these are limited to capturing more quantitative data
• Need for provision that a single data collector to record responses of multiple individuals in separate forms. (Many online forms restrict submission of a single form to a single user)
• Need for the app/ form to be useable on the phone ( android ). The data collation/ admin / collation sheet/ summary sheet should be viewable on the desktop
• Need for offline data entry
o The individuals who will be collecting this information have basic literacy, tech knowledge and hence we were hoping to use an app/ online form, that would help collate data easily.
o Also, since the data collection teams would not be capable of collating data for analysis, we were hoping the app/ online form would help create automatic summary sheets which would help analysing real time data.
o Since these villages (where data needs to be collected) have low internet accessibility, it would really help if the team could enter the data in the app/ form offline, and then need to use internet only to upload the data (when they reach an area having internet)
• It would really help if language versions are possible i.e. In addition to it being available in eng, if other languages – hindi/ Bengali were possible

I have a couple of questions:
First, does it have to be a mobile app?
Second, what functionalities are you looking at at? Do you simply want the form to send data to a database, and that’s it?
