The Kids Network is looking for a simple and secure data capture form that our mentors can complete after each one to one session with their mentee. 

Volunteer mentors work with their child over the course of a year on a weekly basis. Currently they have complete a word document form after each meeting and email it. This is a lengthy process and has data protection issues. 

Our longer term ambition is to develop an app where volunteers must input their meeting date into a calendar and will get alerted to input their feedback. For now, a simple, secure data capture form which allows easy reporting would be ideal. This would capture both quant and qual data which we could track over time for a number of relationships.
